SAFE ENVIRONMENT

In an effort to create a safe and caring place to learn and grow, Archdiocesan policy requires that anyone wishing to volunteer at St. Madeline parish must have: 1) the required security clearances; and 2) have completed all the necessary Safe Environment trainings. In order to be considered “active” in service to our parish, your documentation must be current. Please return the information needed to Mr. Chris Avery, our Safe Environment Coordinator, at the rectory office as soon as possible in order for you to continue your service to St. Madeline.  For questions and to obtain more information, please contact Chris Avery at [email protected]

Before volunteering, please complete these 6 steps:

Step 1: Criminal Record Check- (valid for 5 years).  This form goes directly to the PA State Police. You must fill in the information and mail it to PA State Police. You can submit electronically and obtain immediate results, but you must print out the certificate and submit to the rectory office. Step 2: Child Abuse Clearance- (valid for 5 years).  This form is completed by you at this site: https://www.compass.state.pa.us/cwis/public/home  Please submit results to the rectory office.

Step 3: Federal Criminal History Background Check

• FBI Fingerprint Clearance for a volunteer who has been a PA resident for less than 10 years.  Code  1KG6ZJ for Volunteers

OR

• A signed Disclosure Form signed by a volunteer who has been a PA resident for the entire last 10 Years and has not committed designated crimes in Pennsylvania or elsewhere. This form is available in the rectory office or can be downloaded by clicking the box below. Please sign and return to the rectory office.

Step 4: Mandated Reporter Acknowledgement Form- This form may be obtained through the school office or can be downloaded by clicking the box below. Please review the mandated reporter information sheet, sign the acknowledgment form, and return to the rectory office.

Step 5: Attend Safe Environment Part 1 ("Protecting God's Children for Adults")

• This training provides basic facts about child sexual abuse and adults’ roles in prevention. This training is held either in-person throughout the Archdiocese or virtually. The available training dates and registration can be found HERE.

STEP 6: Attend Safe Environment Part 2 ("Mandated Reporting Session")

• This training can be completed online and sessions are available on-demand here: www.childyouthprotection.org.
